Created with Fabric.js 1.4.5 Workload Enrolled in Medicaid, CHP+ or KP Step No AnswerNot Interested 245 1,104 Conclusio n The SVVSD Health Insurance Information Form was included in 2012/2013 student registration packets.The form asked whether or not families had insurance, were happy with their existing health coverage, and if health insurance premiums where too difficult to pay. The forms where sorted and prioritized and contact was made to each uninsured family to screen the household members for free or low-cost health insurance. Application assistance was completed with each eligible family for Medicaid, CHP+, or the Kaiser Permanente Step Plan. After contact was completed, the forms were entered into a database. The results of these efforts can be found below: SVVSD Health Insurance Information Project 220 Individuals Indicated No Health Insurance 19% This data is a reflection of the families who returned their Health Insurance Information Form and indicated that they had no access to health insurance. The Health Insurance Information Project not only facilitated outreach and enrollment for many eligible but not enrolled families, but also provided follow up to families already on benefits in need of assistance in accessing health care (selecting providers, scheduling appointments, providing transportation, and much more). This project also shed light on why many families in SVVSD remain uninsured. Some families are over income for public assistance but don't earn enough to pay for private policies. Others don't meet the age, citizenship, or residency requirements of public health insurance programs. Others were just too difficult to get in contact with. In the coming months, we hope toincrease public awareness of our programs and to enroll every eligible child in St Vrain Valley School District.
