Proposed solutions - Use visual aids and plain language and make use of any language services/interpreters in your health center whenever possible - 32 million Americans use 3 or more medicines daily- Half of the adults over 65 have a minimum of 5 prescription drugs, ranging 10-19 pills/day- Forgetfulness, confusion and ambivalence to take the prescribed medications, create a cost of approximately $290 billion in emergency room visits and other avoidable medical expenses- noncompliance results in 125,000 deaths per year SOME BARRIERS TO ADHERENCE Not enough time with doctor/pharmacistduring consultationto review all medications Lack of bilingual staff;hard to understand instructions Complexity: too many medications can be confusing High cost: lack of insurance oraffordable (low-cost)alternatives Accessibility:Pharmacy is too faror not easily accessible Issues trying to reach healthcare providerfor questions regarding prescription - Increase healthcare providers awareness of these and other healthcare literacy issues - Promote the practice of brown bag technique: * patients should bring all medications to each and every doctor/pharmacist appointment, so all medications are reviewed and prescriptions are made accordingly * this might help reduce the number or amount of medications and/or help avoid missuse or overdose - Providers should prescribed medications so that they are all dispensed at one pharmacy, and preferably all at one rather than multiple trips. - Ensure pharmacy is accessible to the patient- Partner with pharmacies that use refill reminder systems - Providers should get informed about prescription assistance programs and offer those to patients or prescribe generic or low-cost medications - It is important to emphasize and understand the long-term benefits of taking medications correctly, and also understand side effects or consequences of non-adherence; write these down if possible - Ensure understanding by using methods such as teach-back or ask-tell-ask: * The patient should be able to explain back to the provider the instructions recieved " The health literacy of a society can be improved both by developing the skills of individuals and by lowering the barriers created by health service personnel and systems.
